The seminar began with an activity of rubbing the palm of one hand with the thumb and spreading out the thumb, and switching to the other hand. After creating some heat rubbing the hands, we put our hands on our neck and had a back massage. The instructor then gave an introduction to the remaining weeks, assignments, activities, and discussion boards. The remaining two weeks (Units 9&10) focuses on the students’ reflections of their desires and professional career vision. Nurses could end up in educational, research, administration, or clinical settings. Students are reminded to check every week’s assignment if they are required to complete a practice experience log alongside the weekly assignments. Regardless of the student’s grade, failure to submit a practice log will result in an “F” score. The focus of the Week 7 assignment is to develop a population health model template that creates a health need for the Population of Interest selected throughout the course in a population health focus. A template is provided for this assessment.
Students will research the population, including their demographics. Identify the gaps in care identified within the community among the POI. Review the risks associated with these gaps. For instance, a gap of lack of education of diabetic patients has a risk of readmission before the window period lapses. Next, students will look into engaging patients in care and treatment. Review including stakeholders and measuring outcomes. Provide an overview that prepares them to complete Week 8 assignment. Always strive to summarize your thoughts in conclusion. The template includes the upstream factors that influence the population of interest, population factors, healthcare system factors, which all influence the ability of the population to maintain adequate holistic healthcare. Discuss the nursing activities that can be used to address the issues identified in the factors and the outcomes from the relationship between the factors and interventions.
Unit 8 assignment entails presenting the information above in a voiceover recorded and attached in the dropbox section. The presentation should be about 15 minutes long. The student should also find a healthcare professional and present the proposal to them. The person could have an experience with the population of interest or have a similar interest with the population. The presentation to the professional should be used to complete an Assignment Feedback Form that is submitted in the assignments section. A template is provided for the presentation. We also discussed health promotion and population health that focuses on complex care management. We contrasted population health with public health.
Prior to this class. I have heard of population health. Some of the reasons we brainstormed for why nurses need to learn the concepts of population health management include an increase in the aging population, focusing on the whole population, preventing illness over the future, and Medicare. I think society spends more money on acute care than population health because the socioeconomic determinants of health drive healthcare to become expensive. Corporations are also focusing on making profits. If people were healthy, nurses, pharmaceutical companies, and hospitals would have no one to attend to them. The population health concerns in my community include addiction, coronary heart disease, obesity, HIV, and lack of hospice care or adequate hospice providers. The seminar was interactive, and students shared different ideas that will be instrumental in the coming weeks.
